Add wrap_element_index_content_element and wrap_converter_index_content_element around *_index_content_element implementations to have a common prototype and be passed or used in entry_tree_element_sort_string.  Pass to index_content_element wrapper to setup_index_entries_sort_strings. Remove converter_index_entry_element_sort_string and entry_tree_element_sort_string.
